Registration (New Student)

Step 5: Prepare for Registration

The below information is MANDATORY to complete the online portion. You will need to upload pdf or scanned copies of the required documents during the online portion of the registration process.

A) Name of Resident School

(Click Locate Your School/InfoFinder to find your school.)

B) Guardian and Emergency Contacts

Three Emergency Contacts with at least one phone number listed are REQUIRED for every student in the district. *Note that each parent/guardian/emergency account MUST also have a distinct email address (an email address cannot be used for multiple contacts).

C: Guardian Identification

(*upload) A passport or valid driver's license. 

D) Residency Verification

(*upload): *3 Residency Verification documents will be needed. Your Deed or Lease must be one of these documents as well as an Affidavit (More clarification on the needed affidavit information is located under Step 4: List of Required Documents) if applicable. Note that the first page and full signature page of Deed or Lease must be sent.

E) Student's Identification

(*upload):  A birth certificate or passport.

F) Student's Doctor Information

Name, address and phone number.

G) Student's Insurance Information

Name of health insurance company.

H) Student's Completed Language Survey

I) Student's Current IEP

(*upload):  If applicable
