School Registration Contacts

After your building secretary has processed the residency portion of your student's online registration, you must complete all mandatory health forms with your child's physician. All complete forms should be sent via email to the school nurse as soon as possible. Click HERE to find the nurse's contact information for your school's school.

Please note:   ALL medical documentation must be received by the School Nurse prior to your child’s attendance at school. If the forms below are not received and approved, your child will NOT be permitted to attend school.

• WTPS Student Physical Form (ONLY the District form will be accepted)

• Full Record of Immunizations (must be signed and dated by the Doctor)
