Step 4: List of Required Documents

A) Student Identification

You will need to upload your child's original birth certificate or another official identifying document such as a passport that includes your child's date of birth.  

B) Parent/Guardian Identification

The legal guardian registering the student will need to upload personal identification. 

C) Residency Verification

You MUST upload evidence of personal attachment to your Wayne address.

  • Your Deed or Lease must be one of these documents as well as an Affidavit if applicable.

  • Note that the first page and full signature page of Deed or Lease must be sent.

D) Acceptable Residency Documents

E. Proof of Eligibility

  1. The district shall require multiple documents of any of the following or similar forms of documentation from persons attempting to demonstrate a pupil’s eligibility for enrollment in the district: Proof of Eligibility (See Acceptable Proof of Eligibility Documents below)

  2. The district may accept forms of documentation not listed above, and shall not automatically exclude from consideration any documentation or information presented by a person seeking to enroll a pupil.

  3. The district shall consider the totality of information and documentation offered by an applicant, and shall not deny enrollment based on failure to provide a particular form of documentation, or a particular subset of documents, without regard to other evidence presented.
